VPK Provider Forms (cont.)
Once the forms are complete, it will be necessary to gather supporting documentation. This documentation includes:
- Certificate of Accreditation (if applicable)
- Copy of the Director’s Credential
- Educational credentials for all VPK instructors
- Proof of the 5.0 hours literacy training attended by VPK instructors
- Level II background screening information for all VPK staff
- FBI letter *
- FDLE report *
- Local screening *
- Affidavit of Good Moral Character **
- Verification of work history

This packet must be mailed or delivered to the identified agency in your community. Check with your local coalition for the submission information in your area. The coalition will notify you within approximately thirty days regarding approval to be a VPK provider. There is no deadline to submit an application to become a VPK provider. However, the provider must comply with the requirement that the program is comprised of 540 hours during the school year or 300 hours during the summer.
The next step is to sign a provider agreement. The Statewide Provider Agreement certifies program eligibility and acknowledges the provider's understanding of the policies and procedures that govern the VPK program. This form is updated annually and will be reviewed in greater depth in the next section.
